Sul Ross State University student-athletes can now access their medical clearance forms in Teamworks.
There are a total of 10 forms (All part of the Medical Clearance Forms Packet) to be completed and electronically signed.

  2. Authorization for Disclosure of Protected Health Information
  3. Consent to Treat
  4. SRSU Concussion Management Policy
  5. NCAA Concussion Acknowledgement
  6. Drug Testing Acknowledgement
  7. Sickle Cell Trait Information and Upload
  8. SRSU Medical Insurance Information and Acknowledgement
  9. Insurance Card Upload
  10. Physical Upload
Important Notes:
  • Medical clearance forms for all SRSU student-athletes are due BEFORE you can begin working out with your team.  
  • Physical exams moving forward after the fall of 2025 must be completed on the SRSU Athletic Examination Form.
    • Physicals must be completed ANNUALLY
    • Your physical must be signed by a provider. We cannot accept physicals forms signed by a nurse or chiropractor. It is your responsibility to make sure that the form is signed by the provider before you leave the provider’s office.
  • All incoming (new and transfer) student-athletes will need to provide documentation reporting their sickle cell trait status. SCT status can be located a few different ways:
    • Contact the hospital you were born at. This is a standard test done at birth that is part of your Newborn Screening.  
    • Ask your pediatrician or PCP to request your Newborn Screening Results.
    • Get Tested
      • You can request testing through your PCP or through a lab in your area.
  • To compete in athletics at Sul Ross State University, you must carry a primary insurance policy.
